Eligibility Criteria for Intellectual Disability Services

  1. A Full Scale IQ (FSIQ) score of 70 or below.
  2. A Diagnosis of ID (Mild, Moderate, Severe, or Profound) and/or a Diagnosis of Autism Spectrum Disorder (ASD). The ID diagnosis needs to be made prior to the age of 22 or it must be documented that the diagnosis most likely occurred prior to the age of 22.
  3. A standardized Adaptive Assessment that displays at least 3 deficits in specific areas of daily living skills including:
  • Self-Care
  • Receptive and Expressive Language
  • Learning
  • Mobility
  • Self-Direction
  • Capacity for Independent Living

*Standardized Assessments for Adaptive Skills include: 

- Vineland Adaptive Behavior Scales-III

- Scales of Independent Behavior (SIB-R)

- Adaptive Behavior Assessment System (ABAS-III)

  1. Must have Medical Assistance.
  2. A copy of a current Physical (completed within the last year) with an Intellectual Disability (ID) and/or Autism Spectrum Disorder (ASD) diagnosis with the required ICF/OMR Level of Care determination denoted.
